Living with chronic joint pain can stop you from enjoying the activities you love. If conservative treatments no longer provide relief, your doctor might recommend joint replacement surgery. Traditionally, this meant a hospital stay of several days. However, thanks to incredible advancements in surgical techniques and pain management, many patients can now have their joint replaced and return to the comfort of their own home on the same day.
This procedure, known as outpatient joint replacement or same-day joint replacement, is a safe and effective option for many people needing a hip, knee, or shoulder replacement. At California Pacific Orthopaedics, our team of board-certified surgeons are leaders in the field, helping patients throughout San Francisco and the Bay Area get back on their feet faster.
Why is Outpatient Joint Replacement Done?
The main reason for any joint replacement surgery, whether inpatient or outpatient, is to relieve pain and restore function to a joint that has been severely damaged, most commonly by arthritis.
- Osteoarthritis: Often called “wear-and-tear” arthritis, this is the most common form. The protective cartilage that cushions the ends of your bones wears down over time, leading to pain, stiffness, and swelling.
- Rheumatoid Arthritis: This is an autoimmune disease where your body’s own immune system attacks the lining of your joints, causing painful inflammation that can eventually destroy the cartilage and bone.
- Post-Traumatic Arthritis: This can develop after a serious injury, like a bone fracture or a torn ligament, which damages the joint surface and can lead to cartilage breakdown years later.
When these conditions cause pain that interferes with your daily life – making it hard to walk, climb stairs, or even sleep – and non-surgical treatments like physical therapy, injections, or medications are no longer working, a joint replacement, or arthroplasty, may be the best solution.
The goal of outpatient joint replacement is to provide all the benefits of a traditional replacement—less pain, more mobility, and a better quality of life—with the added advantages of a same-day procedure. These benefits often include:
- A more comfortable recovery in your own home.
- A lower risk of hospital-acquired infections.
- A faster return to your normal routine.
Who is a Good Candidate for Same-Day Surgery?
While outpatient joint replacement is a fantastic option, it isn’t suitable for everyone. The most important factor is your overall health and safety. Your surgeon at California Pacific Orthopaedics will conduct a thorough evaluation to determine if a same-day procedure is right for you.
Generally, ideal candidates for outpatient joint replacement are:
- In Good Overall Health: Patients without serious medical conditions like severe heart or lung disease, uncontrolled diabetes, or a history of blood clots are typically better candidates.
- Motivated and Committed: A successful recovery depends on you. Patients who are motivated to actively participate in their recovery and follow their physical therapy plan tend to have the best outcomes.
- Have a Strong Support System: You will need help at home for the first few days after surgery. A dedicated “coach”—a family member, friend, or caregiver—who can help with meals, transportation, and daily tasks is essential for a safe and smooth recovery.
Age is not necessarily a deciding factor; it’s more about your overall health and fitness level.
What to Expect with Outpatient Joint Replacement
The journey to a new joint is a partnership between you and your orthopedic team. Here’s a look at what you can expect before, during, and after your procedure at California Pacific Orthopaedics.
Before Your Procedure
Preparation is key to a successful outpatient joint replacement. In the weeks leading up to your surgery, you will:
- Undergo a Medical Evaluation: Your surgeon will order tests to ensure you are healthy enough for surgery. This may include blood tests, an EKG, and a visit with your primary care physician.
- Have a phone call with the pre-op nurse and physical therapist: We believe in empowering our patients through education. You will learn everything you need to know about your procedure, what to expect on the day of surgery, and how to prepare for your recovery at home.
- Prepare Your Home: You’ll want to set up a “recovery station” on the main floor of your home. This might involve moving a bed, setting up a comfortable chair, and removing any tripping hazards like throw rugs or electrical cords. We will also help you arrange for any necessary equipment, such as a walker, raised toilet seat, or shower chair.
- Arrange Your Support: Confirm your post-surgery support person and make sure they understand their role in your recovery.
During Your Procedure
On the day of your surgery, you will arrive at the surgery center a few hours before your scheduled procedure.
- Anesthesia: You will meet with the anesthesiologist to discuss the best plan for you. Most outpatient joint replacements are performed using regional anesthesia, like a spinal block or nerve block. This numbs your body from the waist down, so you won’t feel anything during the surgery. You may also be given a sedative to help you relax or sleep. This approach avoids the side effects of general anesthesia and provides excellent pain control for hours after the surgery.
- Surgery: Your California Pacific Orthopaedics surgeon will use minimally invasive techniques to perform the joint replacement. This involves making smaller incisions and using specialized instruments to reduce damage to the surrounding muscles and tissues, which helps speed up recovery. The damaged parts of your joint will be removed and replaced with a new artificial joint, or prosthesis, typically made of metal, plastic, and ceramic components. The entire surgery usually takes about one to two hours.
After Your Procedure
Your recovery begins the moment your surgery is over.
- Immediate Recovery: After surgery, you will be taken to a recovery area where our nursing staff will monitor you closely. As the anesthesia wears off, you’ll be encouraged to eat, drink, and get up and moving with the help of a physical therapist. The therapist will teach you how to safely walk with a walker or crutches, get in and out of bed, go up and down the stairs safely, and use the bathroom. You will be able to put full weight on the operative leg after surgery.
- Going Home: Once you meet certain goals—such as having your pain under control and being able to walk safely—you will be discharged to go home, usually within a few hours of your surgery.
- Recovery at Home: The first few days at home are focused on rest, managing swelling with ice, and taking your pain medication as prescribed. A home healthphysical therapist will visit you 2-3 days after surgery to check on you and start your in-home therapy. You will continue with physical therapy for several weeks to regain your strength, flexibility, and range of motion.
Most patients are able to walk without a walker or cane within a few weeks and can return to many of their normal activities within six to twelve weeks. Your surgeon will follow your progress closely to ensure you are healing well.
